Home Health Care provides the services of licensed practical nurses (LPNs) for disabled clients in the Twin Cities metropolitan area of Minneapolis / St. Paul, Minnesota.

LPNs provide skilled nursing services including wound care, medication administration including injections and intravenous therapy, and condition assessments including checking vital signs of disabled clients in the Twin Cities. LPNs work under the supervision and direction of registered nurses, and report to registered nurses and physicians.

In addition to providing direct patient care, LPNs also work with disabled clients and their family members and other caregivers in Minneapolis / St. Paul to help them learn ways to maximize the client’s health. This can include teaching how to manage the disease or condition (diabetes, for example), how to help the client care for himself or herself, how to determine when medication is needed, how to know when to consult a member of the medical team, how to dress a wound, how to identify signs of a problem and how to troubleshoot and care for a catheter. In some instances, an LPN provides respite care for the disabled client’s primary caregiver or family member.
