Our staff of nurses, therapists, and aides provides premium home health care to residents of the greater Minneapolis area including the counties of Hennepin, Dakota, Ramsey, Isanti, Sherburne, Anoka, Carver, Scott, Wright.
Home health Care is a formal, regulated program of care delivered by a variety of health care professionals in the patient's home. It is also a Medicare benefit, provided certain requirements are met. For many reasons, the need for home health care has grown rapidly in the past decade. Typical services delivered to the patient in their home include Skilled Nursing, Physical Therapy, Home Health Aide, Medical Social Services, and Durable Medical Equipment.
Home Health Care is one of many State-licensed and Medicare-certified agencies to choose from in greater Minneapolis. When a patient is eligible and interested to receive home healthcare services, one must choose an agency that would provide services. Assistance in choosing an agency may be obtained from doctors, case managers, social workers, or even family and friends. Ultimately, the choice is that of the patient.

Our founder and administrator founded HHC after serving 12 years with the Minneapolis Department of Health, where she focused on the improvement of quality in the Minneapolis nursing home system.
FIRST, we have a commitment to provide premium healthcare services to the greater Minneapolis area, where we have worked with over 500 physicians and 30 hospitals since 1994. SECOND, we have a proven track record of patient improvement, recognized by a recent study by the Centers for Medicare & Medicaid Services (CMS), which listed Home Health Care among the top 45 (of 300) providers in Illinois in terms of patient health improvement in three categories. THIRD, we provide a comprehensive list of services available for our patients, delivered by field caregivers who live in all parts of our service area, whether it be the north, south, east, or west side of greater Minneapolis
When a client qualifies for home care services, Medicare will cover 100% of home care services (i.e. Nursing, Physical Therapy, Home Health Aide, all other therapies, and Medical Social Services) and 80% of the approved amount for Durable Medical Equipment (i.e. wheelchair, hospital bed, walker, glucometer). Medicaid (Public Aid) covers qualified clients for services as well. Most private insurances also cover qualified home care services..
Typically, a patient will be referred to ALLIANCE Home Healthcare by a physician, hospital discharge planner, social worker, or registered nurse. However, anyone may refer a patient to ALLIANCE Home Healthcare. At this point, we will dispatch a nurse to conduct an in-home evaluation to confirm a potential client's eligibility for home care services. All referrals must be under the order of a physician. A patient or family member may also call Alliance Home Healthcare and assistance will be given in obtaining a home care order from the attending physician
